Care Management (aka Aging Life Care) is something that can benefit families at various stages and times of life. Some of the more common scenarios that bring individuals, partners, families, and friends to us for guidance and ongoing support include:
- Long distance caregivers who need the local presence and peace of mind a professional can bring as eyes and ears in support of an older person in their care
- Sandwich caregivers — adult children who are trying their best to care for aging parents as well as their own children while working full time – who need advice on how to balance it all
- A spouse or partner who may be overwhelmed with caregiving responsibilities and needs a break
- A solo ager who is not sure how best to proactively plan for post-retirement and anticipated long-term care needs
- Anyone who is newly diagnosed with a chronic or progressive health condition and could benefit from the guidance of an experienced and knowledgeable Care Manager
- Anyone responsible for the care of an older adult and finds themselves at an impasse because of resistance to care
The answer to when and if Care Management is the best option is dependent on the individual client, their unique needs, goals, and situation.
