Integrated and personalized care is at the core of our work as Aging Life Care Professionals (AKA Care Managers). A person-centered approach is how we best meet the needs of our clients.

There are proven methods for managing symptoms and optimizing care so the person living with Parkinson’s can maintain as much autonomy as possible.

Care management yields positive effects on an individual’s health outcomes, health care utilization, quality of life, and overall well-being, all while addressing the unique needs of their care partners.

Long-Term Planning: Parkinson’s disease is a progressive condition. As Care Managers, we help individuals and their care partners plan for today and be prepared for the long-term. This may include reviewing advanced care planning goals, understanding long-term care options such as home care or supportive housing, and considering end-of-life choices. Since financial and estate planning are essential to the long-term plan, we collaborate with financial and legal advisors to ensure a well-rounded roadmap.

Support and Education: We empower individuals and families to make informed decisions about their care and advocate for their needs effectively through support and education. This includes disease education, care for the caregiver, tailored resource connection and assistance with implementation of physician’s recommendations and guidance.

Physical and Mental Health: Care managers will connect you with the right services and resources to address challenges with speech, walking, balance, and coordination, as well as mental health and social connection.  It is not uncommon for families to be overwhelmed with the coordination of care and communication amongst multiple healthcare providers.  We can be a care liaison to ensure continuity in care and alleviate this burdensome responsibility.

Medication Management: Parkinson’s disease often requires complex medication regimens to manage symptoms effectively. We help clients understand their medications and can implement systems to help with adherence.

Crisis Intervention: We are trained and experienced in recognizing and responding to crises. For individuals living with Parkinson’s, this may involve managing sudden changes in health, coordinating emergency medical care, or providing guidance to family care partners during times of increased stress.

A diagnosis of Parkinson’s disease (PD) can be life-changing, but working with an experienced care manager will provide you with responsive help and deliver best outcomes in your care.
