An Aging Life Care Professional, also known as a geriatric Care Manager, wears many hats and at all times serves as a guide for anyone planning for and/or navigating changes that often come with aging.

We have expertise in aging issues, family systems, communication, navigating health care systems, and local options such as home care, home health, rehab, long-term care housing. We also work collaboratively with elder law attorneys and financial advisors, as well as fiduciaries. We are often utilized as the professional care coordinators.

<What is Aging Life Care?>

Here are a few of the ways we offer guidance and support

  • Serve as consultants to individuals, couples, and families navigating changes that typically happen with aging, whether it be health or memory changes.
  • Help people figure out what resources are available, and what help is needed and when.
  • We have expertise in how to get parents that are reluctance to accept help to accept help.
  • Consult with clients seeking guidance on how to plan for their own aging, being proactive rather than reactive.
  • Sometimes we are engaged when an unexpected health diagnosis or change in health happens. This might include assisting with hospital discharge planning, for example.
  • Prepare individuals, couples, and families who are navigating options for aging well, caring well, and living well.
