Whether it be health crises or weather-related issues and natural disasters, Aging Life Care Professionals™ (aka Care Managers)  are the rock for overwhelmed families by providing calm, creative thinking, flexibility, and excellent communication.

How do Care Managers help families before and in times of crisis? 

Education. Education and understanding are always at the top. Our goal is for clients and families to be well-informed and feel prepared and confident. Clients benefit from our expertise, knowledge and experience in the field of aging and can depend on us for accurate, relevant and reliable information.

Communication. We update and fully inform families about client visits, appointments, and changes of any kind. We also check in with clients by phone or talk directly with staff who are overseeing their care. If an older adult lives locally, but adult children are living elsewhere, we can step in as “eyes and ears,” overseeing supports, coordinating care and advocating for their loved one, even remotely. 

Collaboration. We work collaboratively with allied professionals as well, be they elder law attorneys, housing professionals, placement specialists, home- and community-based supports, or health care providers. Teamwork and collaboration are at the heart of our work and a large part of our strength as a practice. No one is ever alone.

Resources. As Care Managers, we know local, trusted and dependable resources like the back of our hands. And because each client is unique, with individual concerns and circumstances, we know the Who, What and Where of providers, supplies, and supports, matching resources to each client’s unique needs, goals and situation. We do the heavy lifting, often serving as the coordinators of care and coverage, helping balance those needs with overall well-being, so families can focus on being family.

Over the years we have been in practice, we’ve developed strong relationships with allied professionals. We also have the strength of our Aging Life Care network through the Aging Life Care Association. It’s not unusual to have someone locally contact us on behalf of an older loved one in another city or state. We can rely on our peer network for the best referrals in other parts of the country.

In times of crisis, Care Managers offer a guiding light, clear communication, and connection. With our experience comes a solid sense of how best to plan for and navigate what is ahead. We can cut through the clutter of distractions and barriers to forge a clear path forward. We are here to be the calm in midst of the confusion and help clients and their families to live well, care well, and age well.
